The tender passport asks the bidder to declare whether it or its representatives have relevant convictions, including for participation in a criminal organisation, corruption, fraud, terrorist offences, money laundering, terrorist financing, child labour or human trafficking. It also covers unpaid taxes or social security contributions; bankruptcy, insolvency, liquidation or suspended business activity; serious professional misconduct; agreements intended to distort competition; conflicts of interest or involvement in preparing the procurement; serious or repeated breaches of previous contracts; false or misleading information, failure to provide information or requested documents, and attempts to obtain confidential information improperly. Declarations also cover breaches of environmental, social or labour obligations, enabling unlawful employment of foreign nationals, tax offences, and whether awarding the contract would breach international sanctions. Where a ground applies, the bidder may provide information about corrective measures where permitted.

The provided documents state no specific turnover, financial-capacity, prior-experience or staffing qualification thresholds. The bidder must confirm that its offer accepts the tender-document requirements and is not conditional; provide the proposed contract signatory’s name, title and authority to sign (including a power of attorney where applicable), and the contract-performance contact’s name, telephone number and email address; and submit prices in the prescribed pricing structure. The offer must include the completed technical-description and offer form, product descriptions in Estonian and/or English, and manufacturer documentation enabling the hospital to verify each product’s characteristics and compliance. Submit one large and one small toilet-paper roll as samples before the bid-opening deadline; the samples need not be in original packaging, but their packaging must show the procurement title, reference number and bidder’s name. The paper must fit the hospital’s dispensers or the bidder must supply suitable dispensers at no extra charge. If equivalence is relied upon, explain it and provide supporting evidence. Identify any claimed trade secrets and give reasons, but do not classify the price or other protected evaluation figures as trade secrets. The bidder must also confirm that the goods are not subject to international sanctions and that prohibited Russian-linked subcontractors or suppliers will not be used for more than 10% of the contract value.
