Well-being coaching – Well-being on Prescription

Summary

The tender concerns a four-year framework agreement with one contractor for the provision of well-being coaching services from 1 January 2027. The service covers the implementation of Well-being on Prescription and activities aimed at preventing overweight within the Vital Groningen programme. The estimated value is €2,660,000 excluding VAT, and one contractor will be selected. The bidder must submit one reference project demonstrating at least one year of relevant experience. The reference must have been performed within the last three years and have a value of at least €332,500 for one year. It must demonstrate experience in social or well-being services, exploratory or integrated discussions with residents, and cooperation with general practitioners, primary care providers or social-domain partners. Well-being coaches must have at least two years of experience in the social and community field, HBO-level qualifications, knowledge of social and care networks, and experience with the Positive Health approach. Training in the Well-being on Prescription method, participation in intervision, and a valid certificate of good conduct are required; the latter must be renewed at least every five years. The service must be delivered for at least 47 weeks per year. The first conversation must take place within two weeks after referral by a general practitioner or practice support worker. The normal support volume is up to six hours and five to seven conversations per resident, and a trajectory normally lasts no longer than three months. The service must be available in all general practices in the municipality of Het Hogeland for residents aged 18 and over. The bidder is responsible for continuity, reporting, complaint handling, warm handovers, taking over ongoing trajectories on 1 January 2027, and fulfilling the social return obligation. Tenders must be submitted electronically in Dutch. The hourly rate accounts for 30% of the evaluation and quality for 70%; the maximum hourly rate is €110 and must include all service delivery costs.
