The bidder must confirm that none of the exclusion grounds set out in subsections 1 and 4 of § 95 of the Public Procurement Act apply to it. This confirmation applies to all lots. Any further details of the exclusion grounds should be checked in the tender documents if necessary.

The bidder must have completed at least two contracts comparable or equivalent to the subject of the tender, with the contracts completed during the 36 months preceding the commencement of the procurement procedure. This requirement applies to Lots 1–4: the diagnostic and image management solution, lasers, ophthalmology and refraction workstations, and the IVI/procedure chair. The bidder must submit a technical description for each offered product, technical data sheets, brochures, user manuals and other documents proving compliance. Where applicable, a CE declaration of conformity or equivalent document must be provided, together with the risk class and electrical safety class. The bidder must be the official representative or authorised reseller of the equipment, or must ensure delivery and warranty service through an authorised partner. The tender must comply with the procurement documents and must be unconditional. Where necessary, an equivalent solution must be explained and evidence of equivalence must be attached. The relevant technical tender for the lot, general compliance confirmations and a tender table must be submitted. The tender price must follow the specified pricing structure. Where applicable, the warranty must last at least two years from delivery, include repairs and a replacement product during warranty repairs, and ensure compliance with the contract requirements at no additional charge. For office IT equipment, the bidder must declare REACH Candidate List substances present above 0.1% by mass and provide evidence of their presence or absence. The bidder must also arrange reuse and recycling of end-of-life equipment in accordance with Directive 2012/19/EU and describe collection, confidential handling and deletion of data carriers, and dismantling. For furniture, the wood must come from legal sources. Evidence of origin, such as an FSC certificate or a FLEGT or CITES licence, must be provided upon delivery. REACH Candidate List substances in furniture must be declared, with evidence of their presence or absence. The furniture must be detachable and repairable using hand tools, and dismantling and repair instructions must be supplied with it. Furniture must have a warranty of at least two years, and spare parts or functionally equivalent elements must remain available for at least two years after delivery.
