The bidder must confirm the mandatory exclusion grounds listed in the tender documents, including participation in a criminal organisation, corruption, fraud, terrorist offences, money laundering and terrorist financing. If an exclusion ground applies, the bidder may have to provide the date, reason, person concerned, any exclusion period set in the judgment, and information on self-cleaning measures. The bidder must have a construction works supervisor at level 5. The bidder must provide the competent person's name, qualification and certificate number, and on request a written consent to participate in the works. The bidder must also show that within the last 60 months it has completed at least one similar job in the area of surface restoration, accepted and/or deemed compliant. For that reference work, the bidder must provide a list of similar contracts with their value, signing dates, information on the other contracting parties and a reference/link to the relevant development plan or strategy, and in public procurement cases the reference numbers.
