Summary

The tender covers individual occupational medicine consultations for employees of the Wieluń County Office as part of the “From Desk to Health” project. The physician will discuss participants’ examination results and provide recommendations on prevention or further diagnosis. The estimated volume is 75 consultations, with one 10-minute appointment per participant; the actual number of participants may range from 30 to 100. Consultations will take place according to an agreed schedule at the Wieluń County Office or at a provider-selected location in Wieluń that ensures confidentiality. The service must be completed by 28 October 2026. The bidder must be legally authorised to provide medical services in Poland and have at least one occupational medicine specialist available. Price accounts for 60% of the evaluation and the social criterion for 40%; full points are awarded for a commitment to employ or assign at least one eligible person to administrative duties for the entire contract period. The bid must be priced on the basis of 75 consultations. No bid security is required.
