Supply of consumables for the TCM-RADIOMETER monitor and acute peritoneal dialysis sets for newborns and infants

The tender covers the supply, in two lots, of consumables for the TCM-RADIOMETER monitor and acute peritoneal dialysis sets for newborns and infants. The contract period is 24 months, and bids may be submitted for both lots. For Lot 1, each delivery must be worth at least PLN 17,916.00 gross, and for Lot 2 at least PLN 10,500.00 gross. A bidder offering for both lots may meet the requirement with two deliveries whose combined required value equals the sum of the lot thresholds. For medical devices, the bidder must confirm that it holds the documents required for placing the products on the Polish market, including current declarations of conformity and notified-body certificates, or explain why those documents do not apply to the product.
