Summary

The procurement covers the supply of dairy products to the food store of Wojewódzkie Centrum Pediatrii “Kubalonka” in Istebna, Poland. The products include, among others, yogurts, kefir, plant-based spreads, butter, milk, mozzarella, other cheeses, curd cheese and cream; quantities and detailed product specifications are set out in the assortment and price form. Deliveries will be made in batches over six months in response to orders. Goods must arrive on working days between 7:00 and 9:00, following telephone orders placed the previous day, and the supplier must unload and carry them into the food store at its own expense. The bid must cover the entire procurement and include the completed price and assortment form. Price accounts for 60% of the evaluation score and weekly delivery frequency for 40%: three deliveries per week score 40 points, two score 20, and one scores 0. Partial bids and a bid guarantee are not required. The bidder must be registered in a business register; no numerical financial or technical-capacity thresholds are set. Products must be authorised for sale and use in Poland, and transport must comply with food-transport and HACCP requirements.
