Procurement of a practitioner model for continuity of primary healthcare

Pirkanmaa Wellbeing Services County is procuring general practitioner services through a practitioner model to improve continuity of primary healthcare. The service covers four physicians providing care to their assigned patients in the areas of Hatanpää and Tammelakeskus social and health centres, working with the centres’ staff and nurses. Services will be delivered at the buyer’s health centres or in premises arranged by the service provider.
