Supply of self-expanding stents for the Vascular Surgery Department of the Provincial Combined Hospital in Kielce

Summary

The procurement covers seven product lines of self-expanding vascular stents for the Vascular Surgery Department of the Provincial Combined Hospital in Kielce. The products include aortic and vena cava stents and various nitinol stents for arterial and venous treatment; quantities and technical specifications are set out in the pricing schedule. Bids must cover the complete package; partial bids are not permitted. Deliveries are to be made to the hospital’s medical store as required over 12 months. If the agreed quantities are not used during that period, the contract may be extended by up to 12 months, but no longer than 24 months from contract signature. Each order must be fulfilled within a maximum of 5 working days. Bidders may offer a shorter delivery time: 4 working days earns 10 points and 3 working days earns 20 points; 5 working days earns no additional points. Price accounts for up to 60 points, delivery time for up to 20 points, and invoice payment period for up to 20 points: 30 days earns no additional points, 45 days earns 10 points, and 60 days earns 20 points. To qualify, the bidder must demonstrate that, during the last three years, it supplied stents and/or stent grafts for vascular surgery to healthcare providers with a combined gross value of at least PLN 180,000. The offered medical devices must meet the technical specifications in the pricing schedule and have the conformity documentation required for marketing and use in Poland. The bid must include the completed offer and pricing forms and technical descriptions or catalogues; conformity declarations and certificates must be provided within 5 working days if requested. The bidder must not be subject to any of the exclusion grounds set out in the tender documents.
