Supply of tafamidis for the cardiomyopathy treatment programme

Summary

The procurement covers the repeated supply of 81 packs of tafamidis 61 mg soft capsules, 30 capsules per pack, to Wojewódzki Szpital Zespolony in Kielce for the B.162 cardiomyopathy treatment programme. The medicine must be legally authorised for sale and listed in Poland’s current reimbursed-medicines list on the tender submission date. Each delivery must be made within 3 working days of the hospital’s written order sent by email. The contract runs for 6 months; if the full quantity has not been ordered by then, it may continue until the stock is exhausted, but for no more than 9 months. Orders will reflect the hospital’s actual needs, so purchase of the full stated quantity is not guaranteed. Where required, bidders must hold a valid authorisation to supply the medicines. The bid must include the signed tender form and completed product and price schedule, together with the required declarations and documents proving representation authority. Price accounts for 80% of the evaluation and the invoice payment period for 20%; the available payment terms are 30, 45 or 60 days. The unit price per pack must not exceed the applicable funding limit. The medicine’s summary of product characteristics must accompany the first delivery.
